Outpatient Mental Health Services

Polk County’s Outpatient Mental Health Program provides a wide range of service and supports. Clients who are new to our services start with an Intake appointment.  At Intake you meet with a qualified mental health professional for a full assessment.  Together you develop an individualized service plan based on your specific needs and goals.  Services may include:

  • Individual Therapy
  • Family Therapy
  • Group Therapy
  • Referrals for:   
    • Community Based Services such as case management or peer support
    • Psychiatric Medicine
    • Higher levels of care (see Intensive Services below)
    • Skills groups

ACT - Assertive Community Treatment

ACT provides intensive, highly integrated, community-based services for people who have a serious mental illness.

Substance Use Treatment Services

Our substance use treatment program provides both outpatient and intensive outpatient treatment.  Services begin with a drug and alcohol assessment to determine level of care needs.  Our highly trained and experienced Certified Drug and Alcohol Counselors specialize in working with you to develop a comprehensive, individualized plan of recovery which may include: 

  • Individual counseling
  • Treatment groups
  • Peer support services 
  • Referrals to higher levels of care, such as detox and residential services

DUII Services

Polk County Behavioral Health is a State-approved provider of DUII Services. We offer both DUII Rehabilitation and DUII Education to individuals who have been referred by an  ADSS (Alcohol and Drug Screening Specialist).

DUII Rehabilitation program is for individuals who  have had a previous DUII, or meet diagnostic criteria for a substance use disorder. Individuals referred to DUII Rehabilitation must:

  • Complete DUII Education 
  • Complete the substance use disorder treatment services deemed necessary by the diagnostic assessment, and 
  • Abstain from use of intoxicants as evidenced by negative urinalysis tests, except as allowed in ORS 813.200, while outside of a controlled environment for no less than 90 days prior to DUII services completion.

DUII Education is for individuals who have had no previous DUIIs and do not meet diagnostic criteria for a substance use disorder. Individuals referred to DUII Education must:

  • Complete a minimum of 12 hours of education over a minimum of 4 sessions over 4 weeks, and 
  • Abstain from use of intoxicants as evidenced by negative urinalysis reports, except as allowed in Oregon Revised Statute (ORS) 813.200.

Telehealth Services

Polk County is pleased to offer Telehealth video appointments to individuals enrolled in services.
