Case Management

The Polk County Developmental Disabilities Program offers case management services to individuals who are eligible for services due to an intellectual disability and/or a developmental disability. This is in accordance with the Oregon Administrative Rule 411-415-0010 through 411-415-0120.

Case management services include, but are not limited to:

• Assessment of support needs
• Developing an ISP or Annual Plan that may include:
  • Authorized services
  • Information and referral for services
  • Monitoring the effectiveness of services and supports
