Assertive Community Treatment (ACT)
Assertive Community Treatment (ACT) is an evidence-based program designed for individuals who live with Severe Mental Illness (SMI), experience symptoms that interfere with essential life functions such as employment, social relationships, independent living, financial management, and physical & mental health maintenance, and have not found traditional office-based services and interventions helpful.
The ACT program offers home and community based services which include:
- Psychiatric Care
- Nurse Support
- Individual Therapy
- Group Therapy
- Case Management
- Peer Support
- Supported Employment
To be considered for admission to the ACT program, a referral form must be submitted.
Referral Process:
Referrals can be sent by a service provider or individuals may self-refer by completing the Universal Referral Form and submit it using any of the methods listed below:

Steps to follow:
- If you are submitting a referral for someone other than yourself, please include an updated release of information (ROI) with the referral documents.
- Complete the Universal ACT Referral Form; please note incomplete information may cause delay in processing time.
- Gather and submit any other supporting documentation that may be helpful to review for making an eligibility determination.
- Contact the Polk County Behavioral Health ACT Team if you have any questions.
Once the referral is received by the ACT Team, it will be reviewed for eligibility. Individuals submitting the referral to ACT may expect to receive outreach from the ACT team lead with updates regarding eligibility, admission or recommended alternatives, and next steps.
