Things to know before you begin:

  • The online registration form is ONLY available if you live in Polk County and have the Oregon Health Plan.

  • The online registration form may take up to 20 minutes to complete.

  • At the beginning of the online form you will be required to open a link to review our Orientation Materials.  We have included links below if you would like to review them prior to starting the online form. 

NOTE: Our Mental Health provider panel is NOT currently accepting new clients at this time. 
Our Substance Use Treatment program remains open for same day access for new clients.
