Polk County conducts site evaluations for septic site approval, issues septic permit specifications when a repair, alteration, or new system is required, provides pre-cover inspections, authorizes the use of an existing septic system when replacing a home, inspects pump trucks annually, and provides homeowners with knowledge of septic system functions and maintenance.

If you are applying for a septic permit or authorization for a property located within the City of Salem Urban Growth Boundary, the City of Salem will need to sign off on a Department of Environmental Quality Land Use Compatibility Statement (LUC). The LUC form will need to be taken to the City of Salem's Permit Application Center located at 555 Liberty Street SE, Room 320, Salem, OR 97301. Once the City of Salem has signed off on the LUC form, please provide a copy with your septic application.
